Basesana is the Milan polyclinic where TELOS was built and refined. Multi-disciplinary (PT, sports medicine, neuro rehab, wellness), team of 14 professionals, ~4,000 active patients/year.

Challenge

Real outcomes invisible. High drop-out on chronic patients (>35%). Difficulty proposing renewals: perceived as "sales".

March 2025 — March 2026: TELOS rollout across the entire team. Unified intake standard (ICHOM-aligned PROMs/PREMs), milestones per clinical cluster, automated weekly recaps for every patient, aggregate dashboard for the director.
